Forms
Before the first class, you must sign and submit the required safety and liability forms. Formerly, those forms were accessible on the Jacobs School of Music website. Now, the only way to access the two required forms is through your Confirmation of Registration which is automatically emailed to you after enrolling with the online registration form.
Only the parent/guardian may complete these forms and submit them to Indiana University using one of the following methods (fax and mail are no longer accepted):
- A parent/guardian may submit forms in person to the Program Director or Assigned Teacher at their child’s first pre-college class of the academic year.
- Before the first class, a parent/guardian may email forms to the secure IU BOX HEALTH folder for their program. The link is provided in the Confirmation of Registration mentioned above. Only email these forms to the IU BOX HEALTH folder, and please attach as a single PDF document with a file name of CHILD’S LAST NAME, CHILD’S FIRST NAME.

Courtesy Discounts for Multiple Siblings or Classes
Courtesy discounts are offered only during the Fall and Spring semesters. Discounts are NOT automatically applied to students’ accounts when this option is chosen but are subject to approval by the Program Director and the Office of Pre-College and Summer Programs. If unsure whether you qualify, contact the Office of Pre-College and Summer Programs: (812) 855-6025 or musicsp@indiana.edu. You may delay payment until any requested discount is approved.
Piano Lessons Discount: Families with 3 or more children enrolled concurrently in the Young Pianists Program are eligible for a $150 discount each semester on piano tuition (one discount per family).
Theory Class Discount: In order to encourage students to learn piano or another instrument to enrich their musical education by taking Theory classes, Young Pianists offers substantial discounts to most families enrolling 2 or more children in Theory classes. Individual students taking two Theory classes concurrently are also eligible for a discount on the second class. Theory discounts are in units of $40. The number of theory discounts a family may claim depends on how many siblings study piano through the Young Pianists, as well as how many theory classes each child takes. Classes must be taken in the same semester to qualify for the discount.
Please read the Table of Theory Class Discounts below carefully.
